A: No. The name on your Claim of the Life is the name on your ID-documents. If your name is different on your passport, driver’s license and/or birth certificate, you may choose the name on the most recent passport OR choose the birth certificate name.


Q: What if I changed my name?
A: The name on your Claim must be the legal-name on your ID-documents and your name-change-documents (your legal-name-change must be past the ~49-day-timeline.). A spiritual name cannot be on your claim unless it is on your name-change-documents and ID-documents.
